Patient Information

For Your Appointment

  • Arrive to your first appointment 30 minutes early to complete new patient information
  • Bring photo ID and your health insurance card
  • Bring a list of all current medications you are taking
  • Bring a copy of any sleep studies, X-rays, MRI, CT scans or lab tests taken in the last year and a copy of any medical records specific to the condition being evaluated. This will help prevent unnecessary duplication of tests
  • Bring a family member to help with questions, especially if there is a hearing or speech barrier, or if the patient is coming from a nursing home
  • Bring written parental authorization if someone other than the parent/legal guardian accompanies a child 0-18 years of age


  • We require a referral from your primary care physician or other specialist


  • We accept most major insurance plans. Please check with your insurance company to see if we are listed on your plan. Please understand your insurance benefits and come prepared to pay your insurance co-pays and/or deductible at the time of your visit.

How to Cancel Your Appointment

  • We request 24 hours notice to reschedule or cancel any appointment. This will allow us time to offer the time slot to other patients. While we understand that unforeseen circumstances occur, we ask that you please respect the time we have reserved just for you. Please note any patient more than 15 minutes late will be considered a no-show. Patients who miss more than two scheduled appointments may be dismissed from the practice.

Covid-19 Operating Procedures

  • In accordance with recommendations from the Kentucky Department of Health, we are open with limited waiting room seating and increased cleaning/disinfecting procedures. All providers will be wearing masks and we ask our patients to do the same.

  • To protect our patients, we are happy to offer telehealth visits via computer or telephone as an alternative to in person visits.

  • We encourage you to continue to use discretion with your visits out of the home as there may be a resurgence of Covid-19 cases with more social interaction. Please wear your mask when you are out of the home.
